Starting a new game on fast difficulty and intentionally failing the first encounter mission has no even popping up, whether I'm not entering the portal or entering it and die in it.

I've just confirmed that for myself as well. It seems people don't really see discussions too well, so I'll go ahead and edit that into her page.
Okay, nevermind, it does trigger in a normal speed run on day 15; I guess it's only a problem in short runs.
It doesn't trigger on a long run either.
I just tried a short run and that didn't trigger either. Guess it only happens in quick runs now?
... And now it triggers on a normal length run, when it didn't before? What magic is this?
The event probably triggers after a random number of days, and the length settings might mess with that number; it could also be that other events (like Avy joining) interfere with this event triggering.
I've just got it to trigger in all modes. Usually 1-5 days after losing the First Encounter.
Requirements seem to be:
- Playing as Creation
- After day 10
- Lost/Ignored the FirstEncounter
After meeting all the requirements there's ~25% chance of the event triggering at the Home location.
Wait, it's chance-based?! I never knew that.
